Abstract

Relevance. The article presents the results of surgical treatment of 42 adult and elderly patients with cholelithiasis complicated by acute calculous cholecystitis and choledocholithiasis, analyzes the timing of hospitalization, clinical features of the disease, the structure of concomitant pathology, the amount of preoperative preparation and surgical tactics. The clinical picture of acute cholecystitis in patients of older age groups is characterized by pronounced intoxication with the disappearance of local symptoms and a high incidence of concomitant diseases. Short-term preoperative preparation and differentiated timing of surgery reduce the risk of postoperative complications. The introduction of modern diagnostic and treatment methods has improved the results of surgical treatment of elderly and senile patients, as well as the modern method of diagnosing these complications is to assess the general condition of the patient, which allows you to choose the optimal treatment method: laparoscopic or laparotomy. Despite the advantages of laparoscopic cholecystectomy, it cannot be performed in the elderly due to the presence of concomitant diseases, since the use of pneumoperitoneum has a negative effect on the cardiovascular and respiratory systems due to increased intra-abdominal pressure.
